The occurrence of hemorrhoids can be attributed to a set of different causes. In case of constipation or diarrhea it can come to excessive straining during bowel movement, which may lead to hemorrhoids. Especially women with premenstrual syndrome or menstruation experiencing constipation due to water retention are mostly affected by the occurrence of hemorrhoids.
A special kind of hypertension, so called portal hypertension, can also be a factor for the buildup of hemorrhoids. One side of the portocaval anastomoses occurs in the rectal wall. In case of portal hypertension the draining blood gets stuck-up and dilates the smaller veins in the rectal area. Obesity, often a deciding factor for hypertension and diabetes, can also increase venous pressure in this area. Additionally weak muscle tonus and a poor posture may also result in an increased pressure on the rectal veins. Pregnancy may induce the buildup of hemorrhoids through development of hypertension and increased strain during bowel movements. Another prominent reason is an insufficient liquid consumption, which leads often to a hardening of the stool. Continued lack of sufficient liquid intake may lead to chronic constipation, which likely induces the buildup of hemorrhoids. But also foods, like yogurt, or other dairy products, which contain high amounts of lactic acid, can lead to the development of the disease. Vitamin E deficiency is also regarded as a fairly common reason for hemorrhoids.
A rather unusual possible reason was observed Dr. Berko Sikirov. He saw a correlation between the occurrence of hemorrhoids and the use of “sitting” toilets, instead of squatting toilets. In a study in 1987 he tried to confirm his hypothesis that a continued used of squatting toilets will help resolve the hemorrhoids problem of the patients. The results showed that 18 out of 20 experienced complete relief of symptoms, like pain or bleeding, with no recurrence up to 30 months after the end of the trial. Yet the number of probands was to small to be of any statistical significance. Nevertheless it served as an interesting outlook into a completely different direction for a cure. Although the outcome was positive and needed only more confirmation by a bigger number of patients to be included into a similar study, no such has ever been conducted. But squatting instead of sitting is meanwhile generally accepted as possible cure and prevention of hemorrhoids.
